Medicare beneficiaries may enroll in the plan (except Dual Eligible SNP plans) only during specific times of the year.

Thank you for your interest in MD Care Healthplan. You may enroll by doing any of the following:

1. Read the following instructions and click on the Enroll Now button below.

Clicking on Enroll Now will take you directly to the Centers for Medicare and Medicaid Services (CMS) Website.
Please scroll down to the section titled SEARCH TOOL and select Compare Health Plans and Medigap Policies in Your Area.
Follow the directions as instructed.

 

Medicare beneficiaries may enroll in MD Care Healthplan Advantage I and Advantage Select plans through the CMS Online Enrollment Center, located at http://www.medicare.gov.


2. Make a home appointment by calling 1-888-322-2730 (TTY/TDD: 1-800-735-2929)


3. Request for information via mail by calling 1-888-327-2730 (TTY/TDD: 1-800-735-2929) from October 15th to March 1st between the hours of 8 AM to 8 PM, seven days a week, after March 2nd from 8 AM to 8 PM, Monday through Friday.


4. Download the MD Care Healthplan Membership Application Form (below) and mail to the address below:

Within 7 calendar days of the receipt of your paper enrollment request, MD Care will provide you with either a written notice that acknowledges receipt of your completed enrollment request, or if your application is incomplete MD Care will send you a written request asking for additional information to complete your enrollment or denial of enrollment.
